Clinical bioethics is big business. There are now hundreds of people who bioethics in community and university hospitals, nursing homes, rehabilitation and home care settings, and some who play the role of clinical ethics consultant to transplant teams, managed care companies, and genetic testing firms. Still, there is as much speculation about what clinically active bioethicists actually do as there was ten years ago. Various commentators have pondered the need for training standards, credentials, exams, and malpractice insurance for ethicists engaged (...) in clinical consultation. Much of the discussion seems to accept an implicit presumption that all clinical ethics consultation practices look pretty much alike. But is this accurate? What do clinical ethicists do, how and where do they do it, and what kind of clinical ethics is useful in the hospital and in other settings? (shrink)

At the end of the twenty-eighth chapter of the eighth book of the Natural History Aristotle says: S0009838800018425_inline1, and goes on to narrate a strange story of the method employed to procure the hybrid. Though the details are entirely fabulous, it has not been doubted that the Indian dog was a real animal. In de Generatione Animalium 746a34 he says more cautiously S0009838800018425_inline2 What then was this creature ? Sundevall declines to commit himself. Aubert and Wimmer think perhaps a jackal, (...) for they hold that the θxs1F7Dς of Aristotle was no jackal. But apart from other considerations there is not the slightest reason for saying that θxs1F7Dς does not mean a jackal in Aristotle as much as in other authors; if some of his statements about it are not true, no more are most of his statements about the lion. Yet this absurd notion that the ‘Indian dog’ was perhaps a jackal is adopted in the Berlin Index 419a15. If we look to India for a dog really foreign to the Greeks we are of course at once confronted with the genus Cyon, familiar to all readers of the Jungle Books as the Red Dog; accordingly our problematical Indian has been identified also with this. One would not think that anybody who had ever seen a specimen of Cyon would dream of calling it a cross between a dog and a tiger. It is true, however, that Aristotle believed that the Laconian hound was descended from a cross between dog and fox , and that the ancients were very reckless in making guesses of this kind; so the Arabs say that the cheetah is a ‘breed between the lion and the pard’! The author argues that our best hope for addressing world problems (from climate change to violence, to poverty) is to teach critical thinking through the study of language and all symbol systems. This means removing disciplinary boundaries so that we can focus more effectively on solving common problems. Human survival also depends upon our critical analysis of electronic media and our wise uses of technology. Critical thinking via all symbol systems is more likely to generate humane actions. Therefore, education—not governments (...) or armies or institutions or private corporations—is our best hope. (shrink)

Augustine is the person from the ancient world about whom we know most. He is the author of an intimate masterpiece, the Confessions, which continues to delight its many admirers. In it he writes about his infancy and his schooling in the classics in late Roman North Africa, his remarkable mother, his sexual sins ('Give me chastity, but not yet,' he famously prayed), his time in an outlawed heretical sect, his worldly career and friendships and his gradual return to God. (...) His account of his own eventual conversion is a classic study of anguish, hesitation and what he believes to be God's intervention. It has inspired philosophers, Christian thinkers and monastic followers, but it still leaves readers wondering why exactly Augustine chose to compose a work like none before it. In 1986, philosopher-bioethicist Samuel Gorovitz published an essay entitled “Baiting Bioethics,” in which he reported on various criticisms of bioethics that were “in print, or voiced in and around … the field” at that time, and set forth his assessment of their legitimacy. He gave detailed attention to what he judged to be the particularly fierce and “irresponsible attacks” on “the moral integrity” and soundness of bioethics contained in two papers: “Getting Ethics” by philosopher William Bennett and “Medical Morality Is (...) Not Bioethics,” coauthored by us. Gorovitz attributed some of the criticisms that bioethics was eliciting to the fact that this new, rapidly rising, and increasingly visible field had brought “scholars and practitioners together who otherwise would have little exposure to one another's disciplines. Their interactions are mutually enriching at times,” he declared, “but mutually baffling and even infuriating at other times.” In this latter regard, he suggested that “perhaps” Fox and Swazey's characterization of bioethics in the article he dissected “reflects a general revulsion at endeavors they see as inadequately like the social sciences or insufficiently respectful of them.” He went on to say that despite his objections to our “complaints” about bioethics—especially to our claim that “autonomy [had] been an unduly emphasized value” in the field—he had “a lingering sense” that there might be “a grain of truth” in them. Gorovitz ended his essay with an affirmation about the “benefit” that bioethics can derive from “responsible” and even from “irresponsible” criticism. “The unexamined discipline invites the philosopher's critical scrutiny no less than the unexamined life,” he aphoristically concluded. a. This article explores the principles of interaction that shape grammatical practices of conversational speech cross-linguistically. Seven such principles are explored, and the grammatical practices they give rise to are illustrated. The role of these principles in shaping non-linguistic behavior is also touched on.

Peirce and Mikhail Bakhtin, this paper demonstrates that each thinker’s reliance on a triadic model can be incorporated to explain the analogous relationship between the dialogical movement within the sign vehicle and without it. Inside the sign, the dialogical relationship between the immediate and dynamical objects transposes its form onto what becomes Bakhtin’s dialogical model of consciousness with its centripetal and centrifugalvalences. These are pulls within a consciousness (...) toward internal consistency and external dynamism. Taken together, Peirce’s semeiotic and Bakhtin’s theory of dialogical development offer a nuanced, post-traditional explanation of the shape of self, the dialoguebetween the semiotic form and human consciousness. This article focuses on key features of the use of sex and gender in titles of articles about women, science, and engineering over an important forty-six-year period. The focus is theoretically and empirically consequential. Theoretically, the paper addresses science as a critical case that connects femininity/masculinity to social stratification; and the use of sex and gender as an enduring, analytical issue that reveals perspectives on hierarchies of femininity/masculinity. Empirically, this article identifies the emergence, development, and stabilization of published articles about (...) women, science, and engineering that use sex and gender in their titles. The distinctive method involves search, retrieval, and review of 23,430 articles, using intercoder reliabilities for inclusion/exclusion. This results in a uniquely specified and comprehensive set of articles on our subject and the identification of titles with sex and gender. Findings point to the growth of gender titles, their increase in every field, differing concentrations of sex and gender titles in journals, a span of telling topic areas, and higher citation rates of gender, compared to sex, titles. Broader implications appear in reasons for the growth of gender titles, meanings of topic areas that occur, insights into social inequalities and science policies, and emerging complexities of nonbinary categories of sex/gender. Surveys and extens work that has been done in the past two years on 'tense logic' and is a sequel to the author's book, Time and Modality.

Studies of primate cognition have conclusively shown that humans and apes share a range of basic cognitive abilities. As a corollary, these same studies have also focussed attention on what makes humans unique, and on when and how specifically human cognitive skills evolved. There is widespread agreement that a major distinguishing feature of the human mind is its capacity for causal reasoning. This paper argues that causal cognition originated with the use made of indirect natural signs by early hominins forced (...) to adapt to variable late Miocene and early Pliocene environments; that early hominins evolved an innate tendency to search for such signs and infer their causes; that causal inference required the existence of incipient working memory; and that causal relationships were stored through being integrated into spatial maps to create increasingly complex causal models of the world. A central debate in philosophy and neuroscience pertains to whether PFC activity plays an essential role in the neural basis of consciousness. Neuroimaging and electrophysiology studies have revealed that the contents of conscious perceptual experience can be successfully decoded from PFC activity, but these findings might be confounded by post- perceptual cognitive processes, such as thinking, reasoning, and decision-making, that are not necessary for con- sciousness. To clarify the involvement of the PFC in consciousness, we present a synthesis of research (...) that has used intracranial electrical stimulation (iES) for the causal modulation of neural activity in the human PFC. This research provides compelling evidence that iES of only certain prefrontal regions (i.e., orbitofrontal cortex and anterior cingu- late cortex) reliably perturbs conscious experience. Conversely, stimulation of anterolateral prefrontal sites, often con- sidered crucial in higher-order and global workspace theories of consciousness, seldom elicits any reportable alterations in consciousness. Furthermore, the wide variety of iES-elicited effects in the PFC (e.g., emotions, thoughts, and olfactory and visual hallucinations) exhibits no clear relation to the immediate environment. Therefore, there is no evidence for the kinds of alterations in ongoing perceptual experience that would be predicted by higher-order or global workspace theories. Nevertheless, effects in the orbitofrontal and anterior cingulate cortices suggest a specific role for these PFC subregions in supporting emotional aspects of conscious experience. Overall, this evidence presents a challenge for higher-order and global workspace theories, which commonly point to the PFC as the basis for con- scious perception based on correlative and possibly confounded information. The second part examines theories of intentionality and discusses the relationship between different theories of naming and different accounts of belief.

The relationship between formal logic and general philosophy is discussed under headings such as A Re-examination of Our Tense-Logical Postulates, Modal Logic in the Style of Frege, and Intentional Logic and Indeterminism.

Propositional temporalism is the view that there are temporary propositions: propositions that are true, but not always true. Factual futurism is the view that there are futurist facts: facts that obtain, but that will at some point not obtain. Most A-theoretic views in the philosophy of time are committed both to propositional temporalism and to factual futurism. Mark Richard, Jeffrey King and others have argued that temporary propositions are not fit to be the contents of propositional attitudes, or to be (...) the semantic values of natural language utterances. But these discussions have overlooked another role that the A-theorist’s posits struggle to play: the role of facts in explaining other facts. Focusing on the case of action explanation by reasons, this paper presents the challenge that explanation poses for factual futurism. It then brings that challenge to bear against propositional temporalism and the A-theory more generally. My argument saddles the factual futurist with surprising commitments concerning reasons, facts and explanation. The futurist might accept those commitments and pay the price. The alternative – which I prefer – is to reject factual futurism, and with it the A-theory. (shrink)

The push of biomedical profits and pull of consumer desire for greater happiness and superior performance heralds a robust market in offspring enhancement. There are two reasons we might worry about the reach of commerce into the realm of selective reproduction. The first concern is that for-profit genetic enhancement, under conditions of economic necessity, would exploit the poor, by coercing them, in effect, to part with reproductive material they would prefer not to sell for money, if not for their desperate (...) situation. The second concern is that the market valuation and exchange of sperm, eggs, and embryos would distort the meaning, and degrade the worth, of those procreative goods. I argue that the concern about exploitation does not give reason to resist a market in pre-natal enhancement, but that the concern about degradation does. This degradation concern gives rise to two specific worries: one about altruism and another about commensurability. I conclude by sketching several policy recommendations to regulate the transfer of money in exchange for sperm and eggs with specified characteristics. (shrink)

The bearing of certain psychological doctrines upon ethical theory is important, and has been made use of especially by those who espouse empiricism in Ethics. It is the purpose of this paper to examine some of these leading doctrines and the ethical theory which has been connected with them. In doing so, it is appropriate to select for examination the views of Professor W. McDougall, as expressed principally in his Social Psychology and Outline of Psychology ; and this for two (...) reasons. One is, that these views are significant of much more than the opinions of one man. They may be taken quite fairly as representative, in the main, of a definite body of doctrine concerning the nature and development of moral experience. (shrink)

This is a revised and expanded edition of a seminal work in the logic and philosophy of time, originally published in 1968. Arthur N. Prior (1914-1969) was the founding father of temporal logic, and his book offers an excellent introduction to the fundamental questions in the field. Several important papers have been added to the original selection, as well as a comprehensive bibliography of Prior's work and an illuminating interview with his widow, Mary Prior. In addition, the Polish logic (...) which made Prior's writings difficult for many readers has been replaced by standard logical notation. This new edition will secure the classic status of the book. The relation between Hume’s constructive and skeptical aims has been a central concern for Hume interpreters. Hume’s two definitions of ‘cause’ in the Treatise and first Enquiry apparently represent an important constructive achievement, but this paper argues that the definitions must be understood in terms of Hume’s skepticism. The puzzle I address is simply that Hume gives two definitions rather than one. I use Don Garrett’s interpretation as a foil to develop my alternative skeptical interpretation. Garrett claims the definitions exhibit (...) a general susceptibility to two kinds of definition that all “sense-based concepts” share. Against Garrett, I argue that the definitions express an imperfection Hume finds only in our concept of causation. That imperfection is absent from other sense-based concepts, and prompts skeptical sentiments in Hume’s conclusion to the Treatise’s Book 1. I close by comparing my interpretation with those of Helen Beebee, Stephen Buckle, Galen Strawson and Paul Russell. (shrink)

In 1999, Médecins Sans Frontières set out to explore and demonstrate the feasibility of preventing and treating HIV/AIDS in a so-called resource-poor, economically and socially disadvantaged setting. The first MSF mission to incorporate antiretroviral treatment into its HIV-AIDS-oriented medical program was undertaken in Bangkok. The second project was launched in Khayelitsha where MSF has been providing ARV treatment for persons with HIV/AIDS since May 2001. Khayelitsha is an enclave of some 500,000 inhabitants, most of whom live in corrugated-iron shacks, without (...) running water or electricity. Unemployment is extremely high; crime and violence are rampant. The general prevalence of HIV/AIDS is 26%, measured among pregnant women. The tuberculosis incidence rate is one of the world's highest for open-space sites. Unsurprisingly, TB/HIV coinfection is very high too: 63% of those with TB are also infected with HIV. (shrink)

Contemporary health care often lacks generosity of spirit, even when treatment is most efficient. Too many patients are left unhappy with how they are treated, and too many medical professionals feel estranged from the calling that drew them to medicine. Arthur W. Frank tells the stories of ill people, doctors, and nurses who are restoring generosity to medicine--generosity toward others and to themselves. The Renewal of Generosity evokes medicine as the face-to-face encounter that comes before and after diagnostics, pharmaceuticals, (...) and surgeries. Frank calls upon the Roman emperor Marcus Aurelius, philosopher Emmanuel Levinas, and literary critic Mikhail Bakhtin to reflect on stories of ill people, doctors, and nurses who transform demoralized medicine into caring relationships. Frank shows how generosity is being renewed through dialogue that is more than the exchange of information. Dialogue is an ethic and an ideal for people on both sides of the medical encounter who want to offer more to those they meet and who want their own lives enriched in the process.
